3. Enquiries and appointments

4. Cancellations and rescheduling

If you cannot attend, please tell us as early as possible so the slot can be offered to another patient. 5. Home physiotherapy visits

For home sessions, the patient or family agrees to provide:

safe, reasonable access to the premises at the agreed time;
a clear, adequately lit space, and a firm bed or surface where the patient can lie down;
the presence of a family member or attender where the patient needs assistance;
accurate information about the patient's medical condition, medication and any infection risk;
an environment free of harassment, intoxication or threat to the physiotherapist.

Our physiotherapist may end or decline a session if the environment is unsafe, if the patient's condition requires urgent medical attention, or if treatment would be clinically inappropriate.

6. Fees and payment

Consultation, session and home-visit charges are [to be completed by the practice] and will be confirmed before treatment begins. Home visit charges may vary with distance and the duration of the session.

7. Patient responsibilities
